Qualifications

You will complete an Intake Form at the time of your first appointment to qualify as a new patient at the Clinic.  An eligibility screener will conduct a confidential interview to determine eligibility.  You must provide proof of all income, a tax return and in some cases a Medicaid denial letter.  The screener will schedule an appointment for you with a health professional at the Clinic.

To qualify, you cannot have any health insurance and must meet our income guidelines.

To best manage your care, we ask that you agree to see only our providers and specialists.

Income will be rechecked every 6 months for the first two years and then annually after that.

The Eastern Panhandle Free Clinic reserves the right to terminate services.

How to Become a Patient
Q. How do I become a new patient at the clinic?

 Call the Clinic at (304) 724-6091 and tell the receptionist you want to make a new patient appointment. The Clinic generally doesn't take walk-ins.

Q. What paperwork do I bring with me to my first appointment?

  •  Proof of income for everyone who lives with you in your house.  If you have no income you need to provide a notarized letter of support from the person who provides your housing and food.
  •  A copy of your most recent Federal tax return
  • List of medications from your pharmacy with the costs of the medications
  • Medicine bottles for medications you are on
  • Proof of medical expenses only if you are making payments on a payment plan set up by a doctor's office or hospital.
  • A Medicaid denial letter if you have one

Q. If I qualify to become a patient at the Clinic, will all of my medications always be free?

No.  The Clinic generally will be able to provide you with free medications; however you may occassionally have to pay for a medication that we are not able to fill.

Q. I have Medicare-will the Clinic provide me with prescriptions only?

No.  The Clinic does not duplicate services that are provided by other resources.  Please call and we will gladly refer you to other organizations that provide help.
